GOV.UH Forms is operated by the Government Digital Service, which is part of the Department for Science, Innovation and Technology.
Information collected through a form
The organisation that publishes a form is responsible for the information it asks users to provide and for what happens to that information after submission.
Each live form must provide privacy information for the service it supports. That information should explain the responsible organisation, the purpose for collecting information, how the information is used and the relevant information-rights contact route.
Government Digital Service operates the common Forms platform and processes information as necessary to deliver the form and its submission to the responsible service.
Editor and service account information
When an authorised government user accesses the Forms editor, the service uses their government publishing identity and access information to authenticate them, control access and record administrative activity.
Technical service information
The service records technical information needed to operate, secure and diagnose the platform. This can include request and security logs and information needed to investigate faults or suspected misuse.
Retention and access
The organisation responsible for a public service determines the retention and access requirements for its form submissions. Government Digital Service is responsible for technical service records and common-platform administration records.
Access must be limited to people who need the information for an authorised purpose.
