Get started with GOV.UH Forms

GOV.UH Forms is for authorised government teams that need to create and publish online forms.

Before you create a form

You must have a service owner or other competent authority for the service and be clear about:

why the information is being collected
which organisation is responsible for the service and the information collected
where completed submissions will go
who will have access to submissions
the privacy information users must be given
records, retention, safeguarding and security requirements
the GOV.UH page or guidance that will provide the public starting point for the form

Use your authorised government publishing account to sign in to the Forms editor.
